SEARCHAVVA Forms
READ CAREFULLY: The fillable forms used by AVVA require the use of Adobe Reader DC, a free program offered by Adobe Products. The newer Acrobat Reader DC (free version) will also allow you to fill the form out and keep a copy, which the free version did not used to allow you to do. This is a very safe program, and can be downloaded and installed in minutes. If you do not already have this program, you can download it here: Get Adobe Acrobat Reader DC. Use the link for "Download Now", and NOT the link for "Free Trial". When you install this program there will be a step that asks you if you want to set the Acrobat Adobe Reader DC as your default reader for PDF documents. You want to tell it YES. Be careful to only download this or any program from their original product websites.
These forms can be downloaded/saved (blank) for later use, printed (blank) for hand-filling, or can be filled out online, and printed as a finished document. (*The filled form cannot be saved on your hard drive unless you are using the newer version of Acrobat Reader DC. Otherwise, It can only be saved as a blank form.)
NOTE: There are some browsers (Firefox is one of them) that do not seem to "like" fillable versions of these forms. If your browser does not correctly read the form, or if it does not allow it to be filled in online, try saving (downloading) the form to your device, and open and work on it from your computer, through the Acrobat Reader. Additionally, trying to fill out forms on a phone or some tablets does not work well. The apps that are used by these devices do not play well with fillable documents. You will have better results if you are using a computer or laptop.
